Utah Clears Nolla Health's AI Acne Prescriptions
Utah's AI sandbox lets the Nolla Derm app write topical acne prescriptions, with two doctors signing off on the first 100 patients.
In short
Utah has cleared the startup Nolla Health to let its AI issue prescriptions for topical acne medication on its own inside a state supervision sandbox, while oral drugs and isotretinoin stay off the list.
At a glance
- Nolla Derm is an iOS app: users in Utah photograph their face and the AI rates how severe the acne is.
- Only topical acne creams are in scope; isotretinoin and oral medication are excluded.
- Stage 1: two physicians sign off on every prescription, for at least 100 patients.
- Stage 2: the AI prescribes on its own, with retrospective physician review across 500 patients.
- Stage 3: physicians audit at least 10 percent of prescriptions each month.
A piece of software can now write a prescription in Utah before any doctor has looked at it. Nolla Health's iOS app, Nolla Derm, takes a photo of the user's face, grades the acne, and issues a prescription for topical treatment — no consultation in between.
The guardrails
The permission is deliberately narrow. Creams applied to the skin are allowed; isotretinoin and oral drugs are not. Anything the system does not have authority over is handed to a licensed physician instead.
Oversight taper, in three stages
Stage one keeps two physicians on every single prescription for a minimum of 100 patients. Stage two hands the decision to the AI and moves the physician review after the fact, covering 500 patients. From there, physicians sample at least 10 percent of prescriptions each month — a rate, not a review of every case.
Who signed off
The pilot sits inside the state's AI Learning Lab sandbox. Sign-off came from the Utah Office of Artificial Intelligence Policy, the Utah Division of Professional Licensing, the Utah Medical Association, and the American Academy of Dermatology. Founder Luis Wenus frames it as the AI making the prescribing call itself, and calls it the first autonomous prescribing system in the country.
What is not settled
Chief medical adviser Zaid Fadul points to flu and urinary tract infections as possible next conditions, conditional on the pilot holding up. Pricing, funding, and how a bad call gets handled for the patient are not public. This report rests on a single verifiable source; a second account of the same announcement could not be retrieved at publication time and is not reflected here.
FAQ
Can I use Nolla Health's AI prescribing outside Utah?
No. The clearance applies to Utah's AI Learning Lab sandbox and covers users in that state only.
Which acne drugs can the AI actually prescribe?
Topical creams only. Isotretinoin and oral medication are excluded, and out-of-scope requests go to a licensed physician.
Does a human doctor still check the prescription?
Less and less by design: two physicians per prescription for the first 100 patients, retrospective review across 500 patients, then a monthly audit of at least 10 percent.
